In order to complete the RFQ form, you will need to have
information about your company such as its workers' compensation
policy, state unemployment account(s), and employee benefit
plans. In order to provide you with an accurate and binding
quote, we will need copies of certain documents relating to your
workers' comp policy, loss run history, unemployment claims
history, group insurance plans, and your company's retirement
plan. If you don't have a benefits plan or retirement account,
no worries - just indicate "n/a" and we can provide optional
quotes for those services with our proposal or at a later time.
